Please Note:
We accept monthly payment for treatments over $1000.00 for 90 days. There is a 1% administrative fee for pastdue accounts over 90 days. Monthly payments are required on all balances.
Crystal Lake/Lake Geneva/Huntley/Burlington OMS is an independent practice. For patients with insurance we are happy to work with your carrier to maximize your benefits and provide the documentation you need to receive reimbursement for your treatment.
